Terms & Conditions/Patient Agreement

  • On The Go Phlebotomy (OTG) is an Independent 3rd party convenience service. We do not work with or bill insurance companies.
  • Any billing questions or problems for tests collected for this service should be directed to your insurance company or the laboratory billing department. Payment for services provided by OTG must be collected prior to appointment.
  • OTG is responsible for the drawing and delivery of laboratory specimens. OTG does not collect any specimens without a physician order or a request from a laboratory. OTG is not allowed to give any medical advice, treat, or diagnose anyone.
  • OTG is not responsible for any errors that may occur at the laboratory. It is the receiving labs responsibility to process specimens once delivered by OTG. In the event of a lab error, OTG would gladly return to recollect the sample at a discounted rate. Any error made by OTG will be corrected at no extra charge as long as all correct paperwork is submitted adequately prior to appointment.
  • Please be advised that Mobile Phlebotomy Services is not responsible for any outstanding balances with LabCorp or Quest and will need to be settled prior to a scheduled home lab draw. The labs have the right to decline specimens of patients with outstanding balances.
  • OTG does not receive any test results. If your doctor does not receive results, it is the responsibility of the Doctor’s office or Patient to get in touch with the lab. Please note that some testing may take up to 2 weeks to be completed so please plan your appointment accordingly.
  • Appointments remain tentative until we receive the following: NAME> DOB>COPY OF ORDER>COPY OF INSURANCE>PYMT FOR SERVICES. These items must be received immediately after scheduling an appointment so that we can prepare accordingly. The day before is not acceptable and may result in a cancellation or reschedule.   • WE ARE MOBILE & your appointment time is approximate. We can be a few minutes early or we can be up to 30 mins late due to unforeseen circumstances. Please be patient as we will do our best to arrive on time!
  • At OTG, our phlebotomists are confident and experienced at what they do, however, it may take up to 3 tries at times to collect the required specimens. This is normal, especially with a moving target and the tiniest of veins. We understand how stressful this is for everyone, and we always do our absolute best. The less stress, the higher success rate!
    It is important to keep your child hydrated at least 24 hours prior to the scheduled appointment as well as the morning of. Please wake your child prior to the phlebotomist arriving. Under NO circumstances will we draw the child while asleep.
  • If you know that your child or the patient acts aggressive when upset, are difficult to draw or that it normally takes more than one person to hold them still, it is extremely important that you let us know so that we can take the proper measures for a safe and successful blood draw.
  • OTG cannot and will not draw any without a parent or guardian for safety reasons.
